I think the best one is to send a SIP message indicating that the phone
is not available. This way the calling party does not have to wait until
the call times out to learn that the person does not want to be disturbed.
Mixed mode is interesting but I don't think it is supported by asterisk
natively. One possibility could be to add a new SIP header indicating
that this is a priority call and ignore de DND setting in that case. I
don't know if there is some kind of standard on this respect.
