Intone is re-starting the mplayer process in the background - hence the
lag.

Run intone from the terminal and press the key - Intone will print out
some message like
keyname-Keycode-171, Keycode-171
Send me the keycode with a press and hold.

IMHO, unless FSO decides to handle AVRCP (bt) keypresses separately,
keypresses are generally handled by the app that has focus. How would FSO
know that your key play (on the bt headset) needs to be used by it or by
intone? Unless you define some hotkeys - which wont be sent to the app then.
Perhaps you can open a ticket on the freesmartphone.org trac.

I'm looking at recreating the bug - but am not finding the time right
now. If its an intone issue - I'll sort it out :-) Might take a little while
though.
Thanks for all the suggestions and the testing!
