All OSes we care about do have threads :-)
For those who do not, compat module may implement threads.
That would be an interesting excersise.
Keeping #ifdefs for non-threading version would be a bad idea, imho,
because introducing treads simplifies the design a lot, which gonna lead
to a major code restructuring.
