Com5?! Odd, to say the least. Well, that'd be /dev/ttyS4 or
/dev/ttyS04 under linux. You can make new devices, although since I've
never had to do that I can't tell you exactly how its done (I think it
involces mknod and whatnot). You'll want to do an ls -l /dev/ttyS* to get
an idea what major/minor device numbers you'll need. After that all the
normal procedures should apply, using /dev/whatever as the modem.
Are you sure somethings not odd with the modem? I was under the
impression that modems were generally com 1 or 2, depending on whether or
not you had a serial mouse...

particularly see the section entitled "The Serial Device Special
Files" b/c it has the info on how to add new ones.
Of course configuring dialup can be another fun barrel of monkeys
after this, but that's what the list is for ;-). If you're using telesys
I'll be happy to bundle up all my dialin configs from my lan2ppp slackware
machine (presently unused because we finally have broadband again) for
you.
