Please attach a full "-l 5" log of the first run after USB attach, and another log of the second run as Benjamin Larsson mentioned. Maybe we can see a problem in the logs. Also try different USB ports, USB cables, or PCs if possible please. We've seen weird issues happening due to faulty USB ports and cables.
