I've had a similar experience at most places here. i.e one-refill.
It's either that or no refills. I was in Seattle last year and
waiters/waitresses at most places would fill up my cup without being
asked, to my delight. I don't think it's a policy thing, probably has
more to do with trying to free your table once they're sure you're not
ordering more :)
