It is all to do with what purpose you have in mind i.e. whether you want it to dissolve plaque and fibrin or just digest food. The former you would take it on an empty stomach and the latter with food. This just applies to proteolytic enzymes i.e. protein-eating enzymes which would include bromelain, papain etc/. I believe you have to be careful with nattokinase according to Dr Wong. I believe it is because, in some cases, it can work *too* well. dee
