My guess is simply that "old age" is not something doctors are allowed to enter on a death certificate as a cause of death, so they have to write something else, and when in doubt, they put down "heart stopped' or some such thing. But does that make for another death from heart disease? obviously not.
My mother's death certificate says she died of pneumonia..........in
fact she had had COPD for a couple decades, had an enlarged heart, rigid
damaged lungs (COPD I guess is the cause of that), etc, etc,
etc.........and Binswanger's disease, no thyroid gland (but they took
her completely off thyroid medication anyway! talk about cause of
death)............but when her organs were failing at the last, her
lungs filled with fluid, so they wrote "pneumonia" on the death
certificate.........but to my mind, the pneumonia is just what happens
when everything shuts down...........geez. She was 89, and her heart had
been failing for years, she had severe dropsy..........anybody remember
"dropsy"?
