However, the liver fluke that causes gallbladder cancer is endemic in 
southeast Asia.  I remember dazzling my resident with this information when I 
was an intern, however, I have since purged my brain of the exact species.  I 
don't believe it is F. buski -- it is something-or-other sinensis.  I can't 
think of any other parasite/cancer pairings.

There are multiple viruses that are implicated in cancer pathogenesis.  HPV 
is associated with cervical, penile, and anorectal cancers.  A herpes virus 
has been implicated in Kaposi's sarcoma.  HIV is also associated with KS and 
cervical cancer as well as non-Hodgkin's lymphoma.  Epstein-Barr virus is 
implicated in Burkitt's lymphoma cases occuring in Africa, but not the US.  
Interestingly, a vaccine for HPV to prevent cervical cancer may be available 
in a few years.  I suspect that the alternativistas will disapprove of this 
as well.

Hepatocellular carcinoma (liver cell cancer rather than gallbladder cancer) 
is associated with chronic hepatitis B infections, hepatitis C infections, 
and alcoholic liver disease.  It can probably arise in any setting that 
accelerates the damage and repair cycle in the liver (toxins, autoimmune 
diseases, etc.)

The only bacteria that I can think of that is associated with cancer is 
Helicobacter pylori which seems to increase the risk of gastric cancers.  

Celiac disease is associated with certain lymphomas arising in the gut.  
Again, this is an example of the accelerated damage and repair activity.  
Reflux disease is another damaging agent (i.e. hydrochloric acid) that is 
associated with a risk of esophageal cancer.  Ulcerative colitis has a very 
high incidence of progression to colon cancer.

Off the top of my head, that's all I can come up with, but suspect a few more 
will pop in as soon as I hit the send button.
