People delude themselves and others every day. Some believe they are
following this oath in spite of blatant contradictions between the oath
and their practice of medicine. Others see no moral dilemma because they
have no higher functioning moral "mind" to call them on it. They are
split off and to me, when I attended med school as a grad student in
pharmacology, with 250  med students, some of whom I attended undergrad
school with, there was a definite split or dissociation that happened to
a lot of them right before my eyes. They did not want to study with me
because I needed to be able to reason through stuff and they just wanted
to memorize it and go party.

It just IS this way, call it the human condition. But if we were all
perfectly moral and functional beings do you really think we would have
come back for yet another Earth lifetime? LOL
