When my mail server got slammed, it did forward email on to other
servers, before delivering to some large accounts. Of course that
is when I had the non-deleting queue problem crop up. Any change the
user's account is really large and your queue is really full, and/or
your disk space is really low (and thus fragmented)? Of course,
cranking up the log level to low or all and sending a test message
will reveal everything. Or should.
