Is the first address the actual IP address of the sending server?


Yes. SIMS will use the IP address of the actual connection and the 'HELO $IP' in parentheses when the HELO argument does not resolve to the connecting IP address.

The other meaning of that header is that you are running an outdated version of SIMS.
