Some cable companies and ISP's (such as Cox & Earthlink) block all outgoing 
SMTP except through their servers.  So even with authentication you can 
still only use their outgoing SMTP.  This is infuriating for me because 
even though I run a great SIMS server I have to send my mail out through 
COX which limits me to 5MB attachment size.  I understand why they do it 
(to prevent spammers) but it's still frustrating.

I suppose I could set up SIMS to receive smtp traffic on a different port 
.... Would that require two SIMSes running to continue to support the 
default port?
