I apologize if this is addressed in bis-04. A proxy receives a request with a topmost route header like this : Route: <sip:[EMAIL PROTECTED];maddr=1.2.3.4> where proxy.domain.com is fqdn of proxy. Should it send the request to 1.2.3.4 or ignore the maddr and send the request back to itself, using the request-uri of sip:[EMAIL PROTECTED] ( assuming it does not cause a loop).
