The specification lists those headers that a proxy cannot write in 
Tables 1 and 2. Ones that a proxy cannot write can reasonably be termed 
end-to-end.

It can. There are no fixed rules about a b2bua. Arguably, it may need to 
strip headers, and may need to add them, depending on what it is trying 
to do.

Same as above. A B2BUA, for example, whose job is to interface with 
minimally compliant UAs might strip out any such headers, whereas a 
B2BUA that is more or less meant to be transparent would keep them.
