If you have a NOTIFY pending arrive, the state of the subscription
is pending from that point forward until some other NOTIFY arrives.

It would be very unusual for a refresh subscription to yeild
pending if the subscription were previously active. For that to
happen, the element serving the subscription would have to have
been told it no longer has subscription authorization information for
this watcher. Such an element would most likely send a NOTIFY/terminated
on receipt of that information.
