I believe you are correct, Paul.
The rule about which error is forwarded is that the 'best' one should be
passed on. That is: the numerically lowest error. Eg. 404 is 'better' than
604.

Restating what I think you mean:
A send an invite to P.
P forks the invite to B and C.
B & C both send 180 status back to P and hence to A.
Later, both B and C return errors 3xx-6xx back to P.
The question is what P should return to A, and what should A then do.
Right?
P should return one of the errors, either from B or from C, to A. (I
believe there are rules in 3261 about which value to choose, but I'm not
sure of that.)
The other response is ACKed by P and discarded.
A then is responsible for sending ACK for the response it receives. It
doesn't send a BYE, because the invite has already failed.
