If the proxy added a Record-Route entry,
that would be where the phone would send
the ACK.  The proxy should then route
the ACK to the next entry in the ACK's
Route header.

One additional note, some devices always
or configurably send subsequent requests
within a dialog to their outbound proxies
without requiring the proxy to add a
record-route entry.  If I recall correctly,
this is not per standard.  However it is
potentially better than not sending the ACK
if device is configured to only communicate
directly with its outbound proxy.
