The subscription inherently *includes* identifiers. An example of how this was intended to work at the time is REFER. You have a subscription that is created -- dialog identifiers and all -- using something other than a SUBSCRIBE. If you haven't exchanged enough information via *some* mechanism to have dialog identifiers, then there is no subscription.
