I have been seeing this error in the recent builds. Occasionally, after I add a new user, I see a "permission file replication error", which in turn results in the "permission.xml" file not being updated. And, due to this, the newly added user does not have access to voicemail (403 Forbidden error).. The problem seems intermittent (at least for me), and I have no idea how to reproduce it. Looks like a replication issue to me..
