It did turn out to be a firmware problem. Many thanks to Robert Joly
for spending a lot of time with me to fix the problem and upgrading
firmware. Interestingly, I worked all this time (several months) with
seemingly no problems.

I wonder if device files such as this should be included in sipx (i.e.
bundled and redistributed).
