A couple of tracker changes that I made as part of the above:
* The 4.0 version has been renamed 4.0.0
* All issues that were still open against 4.0.0 (all either Major
severity or lower, or relevant only to the commercial release)
have been set to have Affects Version = 4.0.0 and Fix For =
4.0.1
