I would use the centos net-install.  It's the easiest method for USB.

1. Download the iso image for network install from near mirror server.
CentOS-5.3-i386-netinstall.iso

2. Extract iso file and check in the directory "isolinux". Rename
isolinux.cfg to syslinux.cfg to boot by syslinux.

3. Download the syslinux

4. Extract the syslinux-x.x.zip and copy syslinux.exe to the directory
where can use from the command prompt.

5. Copy all contents in the isolinux directory to the USB memory.

6. Execute the syslinux command like this:
> syslinux -a E:
Note that E drive is the USB memory drive. It should be changed in
your environment.

7. The contents of the USB memory shows as the following:
vmlinuz
syslinux.cfg
splash.lss
rescure.msg
param.msg
options.msg
memtest
isolinux.bin
initrd.img
general.msg
boot.msg
boot.cat

8. Insert the USB memory and change boot device to from the USB memory
at the BIOS setup menu.

9 Install, add sipX repos, and enjoy.
