Here are things that I know can be problems: - iptables is active and TFTP is being blocked - TFTP server is not running - Upgrade Configuration does not specify TFTP or gives the wrong host (This is particularly problematic if your DHCP server insists on giving a different TFTP server address. There is a checkbox to override DHCP's information, but it does not seem to "stick" all the time -- you may have to clear the checkbox before every reboot.)
