Tony, Aside from a SIP over UDP re-assembly issue, I really like bandwidth.com as a provider. They have an awesome portal for manipulating what numbers signal to what servers, they have great rates, and their service is top notch.
They are a 'peer' type provider and control access via firewalls and they don't require any kind of "UA Registration". They don't yet support SIP over TCP, but I know it is in testing/development, so I hope they will soon.
