We use 3.10.2 here and an Ingate. We have siptrunks from bandwidth.com in this 
instance. 
 
1.) When I call internally and hit a user's auto attendant, pressing the 
corresponding key to transfer it to my cellphone shows me the internal users 
caller ID on the phone. 
 
2.) If the call is an external call it does show the external caller id.
 
I get the same thing if the calls are forwarded to the number in the UI for "at 
the same time".
 
I did not have the same results with another ITSP, as they showed me their 
account number on forwarded calls, or unknown/anonymous.

We have a similar situation. I cannot forward the caller-id from the
request because it is tied to the user name of the pbx user.  I simply
strip off the caller id when forwarding the call through sipxrbidge.
Anonymous calling support will be addressed for caller-id soon ( we
have a JIRA issue for it).
