Usually this means the UA is not configured for proper DTMF detection. What 
type of phone/softphone are you using?

Does this happen on internal calls or calls through a gateway? If it happens 
with calls through a gateway, then it is a gateway configuration issue with 
DTMF (improper media type cofigured).
