regarding the leading "7", are you manually dialing "7" and prepending every call through that gateway with a "7' from a phone?
if so, then maybe you should use a custom dialplan for that. prefix 7 + (number of digits) resulting call = dial ___ and append "matched suffix" assign that gateway and don't activate until you place it "above" any dialplan that might also catch it (like a local or LD call), then activate it. I'm thinking whatever gateway you are using isn't able to ignore it.
