As a test - I forwarded another audiocodes 4 port gateway to ext. 111 with the 
same results.  But the configs on both devices are very similar - I'm assuming 
I have a bad item in my config - as such, I'll upload that here as well...
 
I'm hoping it is just something I'm missing...

        just read through this thread... nathaniel, try setting the syslog 
level on the audiocodes to max, do a capture of a bad call, and send the file.  
also, what's the firmware on the audiocodes?

                Well, the Audiocodes still didn't send a PRACK to the 183, but 
in this
                trace the Audiocodes gave up on the call (CANCEL in frame 235).

                indicated that the caller hung up (Reason: Q.850 ;cause=16).
