I dont have a lot of first hand experience (other than testing). AT&T
was excellent. Voxitas worked OK. cbeyond worked ok.

 voip.ms worked well (low cost and quite good).

les.net works for me but I don't know if it would work well for you (
its low cost but I have read some not so good comments on other lists
about them).

voipuser.com is questionable.

It would be good to ask the list for suggestions ( and I see you already have ).

Skype is going to join the list ( we have a template for them ). When
the go public you can try them.
