The reason is that there is a race condition whereby both the phones and the 
SBC are trying  to simultaneously play MOH. Having both enabled is not a 
supported configuration on 4.0.2 the intent is to to support this in 4.2. You 
must disable MOH either on the phones ( recommended ) or on the SBC. Having MOH 
enabled on SBC only is the recommended configuration. If you disable SBC MOH 
you will have dead air for blind transfers via sipxbridge.

Please open an issue against 4.1 and capture and attach the generated log files 
using sipx-snapshot in order to debug this issue.
