How about under User/Permissions - do you have that Dial plan enabled for
the user?

its a 'dial plan' permissions thing.

under dial plan if I have: (enabled, and in this order), it works.
Long distance
International
Toll Free
Voicemail

I copy long distance to 'Utah Out' EXACTLY, and enable permissions of "long
Distance", in this order, still works:
Utah Out
Long distance
International
Toll Free
Voicemail

If I set (in dial plan) permission as "utah out", and user is part of the
utah group, it won't dial.  I get reorder, nothing sent to sip, no packets
to trace, nothing.

even if I leave location 'all' and shared.

even if I change location to 'Utah' and clear shared.

and, just in case, I 'send profiles' to gateway, wait till done and then
start services.

if I ONLY put utah out dial plan (with utah out permissions) in dial plan it
won't dial.
