I don't see the number in your forward (5008073) showing in this log. a siptrace file as xml would show better (as an attachment).
Please ensure the user forward timing is not set for more than 45 seconds. most itsp's disconnect (BYE) unanswered calls in 90 seconds or less. If it is answered right away does it also disconnect?
