In this case I am testing against a yahoo hosted account (IMAP, port 143 is
reachable, etc.).

I've left two messages and deleted on via email and the other via the User
portal, in either case I did not see a message from sipxivr.log that
anything was syncing or changing. I would assume when the XML changes it
syncs "out" to the IMAP server, how does the IMAP server get notified of a
change enacted from it end? How often does the javamailer wait to see if
there is a change on the sipx side?

I am under the impression Yahoo's mail does support IDLE so that should not
be a problem. In the event it did not, I should still see an attempt from
javamailer to delete from the IMAP server once a message was deleted on its
side, correct?
