I think you need to leave "use public address for call setup". This has to do with routing of the messages. Asserted identity has to do with how your itsp wants you to identify yourself. Not all use it. You need to find out what the "value" is they want you to send. If it is not the default, then you can put in what they expect (account number, etc.).
