You need to state two things in that regard:

       routing-table called-e164 {context}
                route .% dest-interface IF_NAME_HERE STRIP_MAP
                route default dest-interface IF_NAME_HERE

        #strip the first digit from outgoing calls
        mapping-table called-e164 to called-e164 STRIP_MAP
          map .(.%) to \1


A routing table and mapping table.
You need for that interface a statement that says to strip a digit. If it is
"specific" your use case needs to provide the specifics needed.

i.e. Only strip calls that are x digits long and start with number "y".  But
all other calls leave alone, etc.

So it really depends on how the system is setup to function right now. Since
all of the calls from that system are received by sipx, it makes more sense
to let the patton strip this, and or analyze this case before passing it on
to sipx, and makes a migration from the old pbx that much easier. My 2
cents.
