We had to remove ALG on our fwall rules. It's been a while now, but I think it resolved the symptom you are having.

I think it might be something to do with how the remote firewall ACK's the call and drops it after 5 minutes because it cannot tell it is established. It might not be anything to do with keepalive, but an ignorant signalling interpretation, but don't take my word for it. I suspect that is what it is.
