I don't see a great deal different in the provisioning logfile on the 
4.2 server. It looks like they both get
"Copied 6 (of 6) Polycom static files from /etc/sipxpbx/polycom."
on the first startup and then
"Copied 0 (of 6) Polycom static files from /etc/sipxpbx/polycom."
on subsequent startups, so that line doesn't seem important.

I can confirm that the exact same Polycom 450 booted successfully and 
auto discovered on the freshly 4.2.0 system.
Is there anything else I can test that would show anything helpful?
I have both systems up on VMware and I can easily toggle between them.
