I can only comment on the issues on 4.0.4. For me, it will work fine for days, and then one day it will deteriorate. First a call will get 'stuck'. It will be parked, and nobody can retrieve it. Through the day it will get worse. More calls will get stuck and eventually no slots are left open. These are experienced users. They are doing it right. Usually I can fix it by restarting the related services. It has been suggested this could occur from networks dropping packets. All my voip networks are isolated VLANs on new cat5e cabling with quality Cisco switches.
