FlowRoute is sending the invite correctly. I did a tcpdump and sent it
to them for analysis. The problem is that FlowRoute does their own NAT
traversal. To get incoming calls to work, I had to uncheck "Server
behind NAT" under Internet Calling-->NAT Traversal.
So the question I have is, can remote workers be configured to work with
these settings?
Also, does the sip domain have to be resolvable on the public Internet
for remote extensions to register? For instance, the host name for my
sipX server is sipx.datatek-sipxecs.com, but that is only resolvable
internally via the DNS server running on the server itself.
Is it possible to have VPN-less remote extensions?

If you did a siptrace it would show there as well.

OK. I'm new to tcpdump so bare with me. Do you need anymore than this to
see if they are sending on 5080?
IP sip.flowroute.com.sip > sipx.datatek-sipxecs.com.5080: SIP, length: 1130

Ditto. Sounds like its not through 5080. PCAP or get a capture at your
firewall and see where the invite is coming in (port 5060 or 5080).
It's either your firewall or the ITSP in both cases. A packet capture
will tell all...
