OK, so you call from an internal extension to leave a voicemail? If so you
are not hitting the audiocodes -- Please better describe your call flow.

If you are running 4.2.1 please verify your repo looks exactly like this:

[sipXecs]
name=sipXecs build service for CentOS $releasever - $basearch
baseurl=http://download.sipfoundry.org/pub/sipXecs/4.2.1/CentOS_$releasever/
enabled=1
gpgcheck=0
<http://download.sipfoundry.org/pub/sipXecs/4.2.1/CentOS_$releasever/enabled=1gpgcheck=0>

If it does not look like that, edit it and do a backup and yum update.
There are two incremental version differences in 4.2.1 and you might
not have the newest one (which is suggested).

Pressing "#" is not necessary to send the message to the VM server. It
should also finish the message and assume if your call disconnect is
properly functioning.

If you call is coming from the PSTN to the VM system, please describe what
steps you took to program the AC gateway. If you created it with sipxconfig,
the section DTMF & Dialing will place all the proper defaults for the AC to
detect DTMF, if you have uploaded and/or burned the config (depending upon
your gateway creation method).

Default: Declare RFC2833 in SDP
