Sorry for the multiple emails.
It looks like this is an actually a g711 vs g729 issue. If the phones are set to use g729, they die.  Sorry for any confusion. I will start an entirely new thread once I put out my many fires...

I attached a merged logfile. I tried creating a trace file, but I couldn't get anything to show up. The logfiles should only contain info about this one call.
In this case 6155008073 is the calling number.
10.81.3.5 is the sipx server.
There is no NAT.
I called 6159253058 which is mapped to ext 200 at IP 10.81.3.254
I tried to do a blind transfer to ext 201 at IP
10.81.3.253
201 rings once, and the call is dropped.

I'm a little slammed, because I'm going to have to roll our office back to 3.1.3c tonight. We had migrated to 3.2.1b. An issue with that is now occurring that I didn't see in testing. Inbound calls cannot be resumed once they have been placed on hold. Yeah, obviously bad testing on my part. I assume maybe it is similar to what is happening above.
I still have the test setup where I can duplicate this, so once I get over our current crisis, I can try and dig back into this.
