Thanks Dave - I have tried both of these - but I guess I must have
messed something else up as it's still not working. Unfortunately I am
away for a few weeks now so I will try on my return to get this to work.

There are two Advanced settings on the Grandstream that can affect
attended transfers.  The first is Enable Call Features.  It must be set
to Yes.  The second is Disable Call-Waiting.  It has been some time
since I have worked with the phone, but I believe that option needs to
be set to No. 

 

I would check those two settings to see if they resolve the problem..

Great! how do i go about getting the sip traces for you?
Sorry I don't know how to - but am new to the whole IPPBX
stuff and trying to understand / work out where everything
is.

And for Michael - the only options I can see for invite are
-
1 Semi-attended Transfer Mode:     RFC5589  or     Send REFER
with early dialog
2 Force INVITE:    No       Yes (Always refresh with INVITE
instead of UPDATE)
3 Check SIP User ID for incoming INVITE:           No       Yes
4 Refer-To Use Target Contact:     No       Yes (not sure of
that one)

And for the server i have 4.2.1 and the phones are running
Program-- 1.2.5.3    Bootloader-- 1.1.6.9
