It looks good to me.
It is very important to decouple LDAP configuration from LDAP attribute
mapping: Adding Apply/Cancel buttons ensure this goal
I also agree to restrict that checkbox only to ldap authentication on/off.
Making it to enable/disable overall LDAP configuration proved to be
confusing for end-users anyway...
