That's where you need an external SBC to come to the rescue.

When we have this NAT issue we really need to talk about 2 different things.
 Remote users / SIP URI dialing vs. sipXbridge.

sipXbridge can live on multiple servers but media relay only happens to a
single outside IP.  So if you have multiple sipXbridges the only thing you
can really do now is NAT them to different outside ports on the same IP
address.

So, I'll go back to where I always go back to.  At this point if you're
serious about SIP trunking or have a HA setup you really need to be looking
at a separate session border controller for things to work properly.  ITSP's
will recognize these major SBC's as certified products and then the ITSP's
will be happy (the big guys are the ones that only want to connect to
certified products...  Acme, Ingate, etc.).
