If you are using a pfsense - make sure that you actually turn on the block 
countries rule.  I originally thought I had it turned on, but I just selected 
the countries to block and didn't enable it...

I was, however, using Tony's recommended rate limiter and I would only get a 
couple call attempts before the firewall started blocking them.  I haven't 
gotten any since I correctly enabled the block countries rule (which has only 
been a month or so).
