                                Two of my friends found that you can access 
this Public Debt Form on line.  The form instructs you to mail it into: 
Treasury Retail Securities, which is the Federal Reserve Bank.  The following 
cite will take you directly to Form 5182.

                                The telephone number (800) 722-2678 is for the 
Bureau of Public Debt.  You telephone them to request Form# 5182.  Complete the 
form according to the instructions and file it into the Federal Reserve Bank 
nearest you.  Your legacy account will be deposited into that Trust and I 
believe you will receive a debit card from the Federal Reserve.
