Skim's ability to go into highlighting/underlining/text note etc mode and stay
there gets you half way to what you want. If you combine it with a macro
program (eg QuicKeys) and a programmable mouse, you can get pretty close to
what you propose.
I have one of my mouse buttons set up to trigger a macro in Skim which types
<Command-5> and then types <H>. This puts Skim into Highlight mode. Any text
selected from now on is highlighted and highlighting stays on until turned off,
so every passage of text selected from now on is highlighted.
Another macro types <Command-5> followed by <U> putting Skim into Underline
mode. I have this set up to pause for just 6 seconds (enough to underline a
single passage) and then type <H> returning me to Highlight Mode.
That happens to suit me, because I highlight frequently and underline only
occasionally. Obviously all kinds of variations are possible.
