PDF Kit is not open source, and there is no API in PDF Kit for setting a page's background color; you'd have to use a subclass of PDFPage and handle all of the drawing. For the page itself, this would be easy using CoreGraphics to draw the -pageRef, but I'm not sure about the annotations.
