Skype 4 menus can be enabled for accessibility by switching Skype to Classic mode, which is a radio button in the General Options page. Jim answered the "how to make a call" question pretty well. You can also make a new call by typing "/call" (that's a slash and then the word call), a space, and the number, such as +13095551212, and pressing Enter, from any chat input window.
